Documents necessary for an animal creamtion

For legislative reasons you wil have to provide the documents which you can download below.  The veterinary will issue Post-mortem report , where he fills in personal data of the owner and the animal, the reason for euthanasia or probable cause of death and the date of the last  rabies vaccination. If you do not want to or cannot attend the cremation, please fill in also the second form, Sworn declaration that your pet  had not hurt anyone seven days prior to its death, which is necessary because of the risk of rabies.
